A written report and personalized exercises are included with every initial evaluation option. Most golf lessons and most medical visits end with a conversation that evaporates by the weekend; the report exists so your findings, priorities and plan survive contact with Monday. The outline below explains what to expect; it is not a patient report or an exercise prescription.
Your goals and findings
The report connects the concerns you brought to the visit with the medical and movement evaluation findings: what was measured, what stood out, which findings matter for your goals and which are simply noted. It explains the priorities in plain language and identifies any questions that need further assessment, so you know both what we found and what we did not find.
Your exercise plan
Personalized exercises with instructions, selected from your evaluation rather than a template, with the reasoning attached: what each exercise is for and what changing it should look like. This is the part of the report golfers use most, and it is written to be usable without translation.
Information for your coach
With your permission, relevant findings can be shared with your existing instructor or the coach providing your included lesson. When instruction is part of the visit, the report records the agreed practice priorities, so the lesson and the physical work point the same direction. Coaches consistently tell us this page is the one they wanted all along.
Follow-up recommendations
The report explains any recommended reassessment or outside care, and why. A movement reassessment is $150 for 30 to 45 minutes when appropriate; the Private Performance Experience includes one scheduled follow up review. Because the findings are measurements, a reassessment can answer whether the plan worked rather than whether it felt like it worked.
